4 in 10 physicians have a side gig

Thirty-eight percent of physicians have outside paid work, according to Medscape’s 2025 “Physician Compensation Report,” published April 11. 

Here are three more stats to know about physician side gigs:

1. Sixty-one percent of surveyed primary care physicians said they have outside paid work, and 62% of specialists, according to Medscape’s survey. 

2. Internal medicine physicians had the highest amount of average secondary income in 2023, with $60,000. Second, was family medicine, who had an average of $54,000, according to Medical Economics’ 2024 Physician Report. 

3. Here are the top 10 sources of secondary income for physicians, according to Medical Economics. 

  • Consulting
  • Medical administrator
  • Clinic work
  • Teaching 
  • Expert witness
  • Market research
  • Hospice
  • Hospital, not emergency department
  • Clinical trials/research
  • Speaking
