A record number of Americans are getting health insurance through the Affordable Care Act, and states that use the HealthCare.gov marketplace are vulnerable to a scheme where plans are switched without the consumer

Federal and state regulators are mulling what they can do to thwart the growing problem. Rogue health insurance brokers are switching consumers’ plans without permission and collecting the commission.
